At FSC, we offer mental health, emotional support animal, ADOS and ADHD testing, and PASRR assessments. Our team is experienced, compassionate, and focused on providing thorough evaluations to help our clients improve their mental and emotional well-being. A basic mental health evaluation helps you understand how the different areas of your life, as well as your past, contribute to both strengths and areas of need in your present situation. This evaluation can identify specific mental health diagnoses and recommend treatment options going forward. Please contact us to ask about our current rates.

EMOTIONAL SUPPORT ANIMAL CONSULTATIONS

Do you feel like a furry friend could improve your mental health? Family Solutions offers consultations to help you in the process of obtaining the needed documentation to qualify for an emotional support animal (ESA). 

ESA consults are currently only offered online a few times a week (feel free to call for more information). The consult can be held in person or through telehealth online where the therapist will complete an assessment and discuss how to best support you. The cost for the consultation and completed ESA letter is $250.

AUTISM DIAGNOSTIC OBSERVATION SCHEDULE TESTING (ADOS-2)

The Autism Diagnostic Observation Schedule™ (ADOS™-2) is a standardized assessment tool used to evaluate communication, social interaction, play, and restricted and repetitive behaviors in children with autism. The assessment involves a trained specialist who directly interacts with the child in social and plays activities to observe and evaluate their behaviors. The cost for ADOS is approximately $800, and a deposit of at least half is typically required to schedule the assessment. The full payment must be made by the time of assessment, which typically takes around 5 hours to complete, including testing and coding for the assessment. Following the assessment, a diagnostic letter will be provided, which may also include recommendations for ADA accommodations.

ADHD TESTING

Diagnosing ADHD in adults involves a comprehensive evaluation by trained practitioners, as there is no single test to diagnose the condition.   During the evaluation process, the trained practitioner may use a standardized interview, as well as ADHD symptom checklists, behavior rating scales, Brown Attention Deficit Disorder scale, Wender Utah rating scale for ADHD, and Executive Skills Questionaire: Dawson and Guare. They may also interview close family members to gather information on the patient's past and current level of functioning. Tests may also be conducted to rule out similar conditions or learning disorders. 
 
The pricing for ADHD testing is typically around $225, and a deposit of at least half is usually required to schedule the assessment, which takes about 2 hours. The full payment must be made by the time of assessment. Following the assessment, a diagnostic letter will be provided, which may include recommendations for ADA accommodations.

At FSC, one of our specialties is providing state ordered mental health evaluations for patients entering nursing homes in Box Elder, Cache and Weber Counties.  We provide accurate, reliable and fast service to help ensure people in skilled nursing care get the mental health assistance they may need.

If you would like to submit for a level 2 PASRR we will need the level 1 completely filled out and signed, a face-sheet, H&P with a review of systems, and a meds list. If you are resubmitting for an end-of-stay we will need the level 1 completely filled out and signed, a face-sheet, H&P with a review of systems, meds list, the last 30 days of progress notes, and the most current MDSs.
